Yes, I wish to enjoy the benefits of membership at 3Rivers Federal Credit Union upon the completion of the merger of West End Bank into 3Rivers Federal Credit Union. By signing this form, I consent to have my accounts at West End Bank transferred to 3Rivers Federal Credit Union ("3Rivers") and to become a member of 3Rivers upon the completion of the merger of West End Bank into 3Rivers. I understand that no additional action is required by me and that the transfer will only occur if the merger of West End Bank into 3Rivers is completed. If this transaction is not completed, this form will have no effect and will be null and void.

An opt-in election by any person (including individuals, partnerships, trusts and corporations) will, unless otherwise indicated, apply to all deposit accounts as to which such person has withdrawal or signature rights.

When signing as attorney, executor, administrator, corporate officer, trustee, guardian, or custodian, please give full title/office.

Deposits at 3Rivers will not be insured by the Federal Deposit Insurance Corporation. Instead, such deposits will be insured by the NCUA.
